A completed Buy American form MUST be filled out and included with your bid to be considered. Offer submissions not including a completed attachment may be rejected as non-responsive. All Shipping/packaging/inspection charges to be included! A single shipment/invoice is requested. Multiple shipments must be approved in writing through the listed POC prior to shipping.
